We want your visit to be as smooth and comfortable as possible. Right now, all new patient forms are provided and completed in the office when you arrive. Our friendly front desk team will guide you through each step to make the process quick and easy.
As we transition to Dentrix Ascend, we’ll be moving to a completely paperless system — allowing you to fill out forms digitally before your appointment for an even faster, more convenient experience.
